- Flexibility to conduct both telehealth and in-field visits based on needs
Description:
This position requires registered nurses to provide telehealth services until they are prepared for face-to-face visits.
Registered nurses must be adaptable to transition between telehealth and in-person assessments as needed.
Candidates should have a minimum of two (2) years of experience in a home setting.
Experience may include working for a certified home health agency, a licensed home care service agency, or managed long-term care.
Conducting assessments similar to the UAS, such as OASIS, is also required.
Providing primary care to homebound individuals and care management for those receiving home and community-based services is necessary.
Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days.
Experience caring for the geriatric population and managing chronic diseases in the home setting is advantageous.
Combination home care experience as a paraprofessional or licensed practical nurse is considered.
Candidates must possess the ability to review health status questions and make evaluations based on UAS criteria.
Effective communication and collaboration with interdisciplinary care team members are essential for improving care coordination.
A comprehensive knowledge of community resources and managed care is required.
Candidates are expected to document findings accurately and maintain a focus on continuity of care.
The role may also involve identifying opportunities for health promotion and illness prevention.
Other related duties may be assigned as needed.
