Evaluations will include assessments of health status, strengths, care needs, and preferences. This will guide the development of individualized long-term care service plans. Candidates must review consumer medical documentation and relevant health referral forms. Evaluation data must be entered into an electronic form and transmitted as required. Any concerns or conflicting information during the evaluation process must be documented. Candidates will emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Discussions with consumers regarding healthcare options, supports needed, and service vendors will occur. Initiating communication with consumers for enrollment into a managed care health plan is required. Enhancing communication and collaboration with interdisciplinary care team members is essential. Candidates will collect quality review data and required documentation for outcome measurements. Maintaining knowledge of community resources, payor requirements, and network services is necessary. Candidates must meet all established performance standards for this position. Additional duties may be assigned by management as required.
