RNs need to be adaptable to shifts between telehealth and in-home assessments based on needs. Candidates must have recent experience conducting comprehensive assessments similar to UAS criteria. Experience can include working with Certified Home Health Agencies, Licensed Home Care Service Agencies, or Managed Long-Term Care. Providing primary care to homebound individuals and managing care for those receiving home and community-based services are acceptable. Experience caring for a geriatric population and managing chronic diseases in the home setting is advantageous. Combination experience as a paraprofessional or Licensed Practical Nurse and RN is also preferred. Job responsibilities include performing initial in-home pre-evaluations of patients. Evaluation data must be entered into electronic forms as required. Any concerns or conflicting information during the evaluation process should be documented appropriately. Continuity of care must be emphasized to reduce fragmentation and duplication in treatment plans. RNs will discuss healthcare options and service vendor choices with consumers.
