The evaluations include assessing health status, strengths, care needs, and preferences. Nurses will guide the development of individualized long-term care service plans. Reviewing consumer medical documentation and relevant health referral forms is required. Evaluation data must be entered into an electronic evaluation form and transmitted as needed. Documentation of any concerns, conflicting information, and other issues during the evaluation process is necessary. Emphasis on continuity of care is crucial to reduce fragmentation, duplication, and gaps in treatment plans. Discussion with consumers about healthcare options, support needs, service vendor options, and waiver options is part of the role. Initiating communication between consumers for enrollment into managed care health plans is expected. Enhancing communication and collaborative relationships with interdisciplinary care team members is essential for improving care coordination. Nurses will collect quality review data and documentation to support outcome measurements. Maintaining a comprehensive working knowledge of community resources and payor requirements is important. All standards established for this position must be met as outlined in the annual performance criteria. Other duties may be assigned by management as necessary.
