Description:
The Home Health Nurse provides safe and competent care to a designated group of patients in their home setting.
Delivers patient care by applying the nursing process based on theory, research, evidence-based practice, and organizational nursing standards.
Assesses and evaluates patient care needs using critical thinking skills.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with patients, prescribing providers, and interdisciplinary team members in the development and implementation of the plan of care.
Provides nursing care according to provider orders, including establishing peripheral intravenous access, administering intravenous therapy, and providing wound care.
Establishes individualized, measurable goals in consultation with patients, families, and healthcare providers to achieve appropriate home health standards of care.
Evaluates progress towards established goals.
Prioritizes aspects of patient care including teaching, coaching, and planning for post-discharge care.
Educates patients and caregivers on their plan of care, promoting health and disease prevention.
Communicates effectively and professionally with patients, families, and all members of the interdisciplinary team.
Considers patients' values, preferences, cultural diversity, and expressed needs in all aspects of care.
Establishes and maintains a therapeutic relationship with patients and families.
Uses evidence-based practices to enhance understanding of patients' perceptions of care.
Initiates service recovery for patients and families who have concerns, escalating to appropriate leadership.
