Job Title: Clinical Care Registered Nurse (CCRN) Profession: Nursing Specialty: Home Health, Geriatric Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM - On-call/Holiday rotations Experience: Minimum of one year of experience working with the frail, elderly, or long-term care population. License: RN Nursing Diploma or Degree from an accredited School of Nursing or University. Certifications: Current CPR certification or willingness to obtain as offered. Must-Have: - Bachelor’s degree preferred in Science of Nursing; Associate’s degree required. - Minimum of one year of home care experience preferred. - Minimum of one-year wound care experience preferred. - Reliable transportation, current valid driver’s license, and car insurance. - Must be 18 years of age or older. - Must be fully vaccinated, or willing to be fully vaccinated against communicable diseases. Description: The Clinical Care Registered Nurse (CCRN) role provides direct nursing care to participants. This role works independently in the application of advanced nursing knowledge and skills. The CCRN is able to manage complex clinical situations. This role collaborates with both licensed and non-licensed personnel. Triaging emergent situations and remaining independent in the application of advanced nursing knowledge is essential. Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs) is necessary. Utilizing care plans independently to provide clinic and home-based direct care is expected. Administering treatments, procedures, and medications as prescribed to participants is a key responsibility. Making home visits regularly to provide care and treatment as ordered by the PCP is required. This may include wound care, lab draws, nurse assessments, and other nursing tasks. The role has a visible presence while working and is subject to frequent interruptions. Communicating with staff, participants, caregivers, and other organizations as necessary is important. The CCRN may be required to work beyond normal working hours and on weekends and holidays. The candidate should demonstrate core values of compassion, integrity, teamwork, and excellence.
