TravelNurseSource is working with Malone Healthcare to find a qualified Home Health RN in Le Mars, Iowa, 51031!
Key Responsibilities- Provide skilled nursing care to patients within their home environment across the designated service area
- Perform OASIS assessments and develop individualized care plans for in-home patients
- Monitor patient progress and adjust care plans as clinically appropriate
- Participate in the on-call rotation, including nights, weekends, and holidays (approximately one in four)
Required Qualifications- Active Registered Nurse (RN) license
- Minimum of 2 years of experience performing OASIS assessments and care planning for in-home patients
- Ability to participate in an on-call rotation covering nights, weekends, and holidays
Preferred Experience / Skills- Prior experience working within the Epic electronic health record system
- Familiarity with home health operations and county-based service delivery models
