Key Responsibilities
Provide skilled nursing care to patients within their home environment across the designated service area
Perform OASIS assessments and develop individualized care plans for in-home patients
Monitor patient progress and adjust care plans as clinically appropriate
Participate in the on-call rotation, including nights, weekends, and holidays (approximately one in four)
Required Qualifications
Active Registered Nurse (RN) license
Minimum of 2 years of experience performing OASIS assessments and care planning for in-home patients
Ability to participate in an on-call rotation covering nights, weekends, and holidays
Preferred Experience / Skills
Prior experience working within the Epic electronic health record system
Familiarity with home health operations and county-based service delivery models
