- Skills in IV management, wound care, assessment, and disease teaching
Description:
This position involves making home visits in various geographic locations.
The average travel per day is between 60 to 80 miles.
Mileage will be reimbursed weekly.
The ideal candidate should be comfortable floating across multiple areas.
Team members are expected to complete a minimum of 6 visits per day, including routine visits, discharges, recertifications, and reassessments.
Patient types include those with chronic diseases, wound care needs, and IV therapies.
Candidates must have experience with OASIS and case management, overseeing up to 25-30 patients simultaneously.
Support within the department includes leadership, schedulers, clinical managers, clinical supervisors, and peers.
Technology used includes EPIC Rover (view only), Focura for documentation, and Samsung Note 9 phones for communication and documentation.
Weekend rotation is not required unless willing to work.
Travelers are expected to work all holidays unless prior approval is obtained.
Travelers should anticipate submitting a request for time off, which requires manager approval.
Team culture emphasizes a supportive environment.
Visit scheduling with patients occurs the day before planned visits.
Cancelled appointments do not count as visit equivalents.
Team members will work towards an expectation of 30 visit equivalents per week.
A company-issued phone will be provided for documentation and communication purposes.
In-person meetings occur weekly, with occasional virtual meetings.
