Description

Key Responsibilities
  • Provide, coordinate, and direct home nursing care in accordance with physician orders and agency policies and procedures through competent application of the nursing process
  • Conduct home visits for a variety of case types including Start of Care, wound care, and follow-up visits, serving approximately 5-8 patients per day
  • Complete all clinical documentation within 24 hours of each patient appointment using the HomeCare HomeBase (HCHB) EMR system
  • Complete and maintain proficiency in OASIS assessments as required for RN clinicians
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses as assigned
  • Travel within an assigned territory of up to 75 miles from the assigned care center, with an estimated 100-150 miles of travel per week
Required Qualifications
  • Active, unencumbered Registered Nurse (RN) license - must be in hand at time of submission; no pending licenses accepted
  • Minimum of 1 full year of recent Adult Home Health experience
  • Current CPR/BLS certification
  • Proficiency in HomeCare HomeBase (HCHB) EMR used within the last 6 months (or willingness to complete a retraining course during compliance)
  • Proficiency in OASIS used within the last 6 months (or willingness to complete a retraining course during compliance)
  • Valid driver's license, reliable personal vehicle, and current auto insurance coverage
Preferred Experience / Skills
  • Experience with a wide variety of home health case types including wound care and start-of-care visits
  • Familiarity with productivity-based scheduling models and ability to consistently meet weekly productivity point expectations
  • Comfort with extended driving between patient homes within a large geographic territory
