Description

Key Responsibilities
  • Provide home visits to patients across the Greater Manchester area, covering a daily caseload of approximately 5-8 patients and meeting a weekly productivity expectation of 30 points.
  • Perform visit types including Start of Care, Wound Care, follow-up visits, and other adult home health services.
  • Travel up to 75 miles from the assigned care center to reach patients; accept all scheduled patients regardless of distance from previous appointment.
  • Complete all clinical documentation within 24 hours of each patient appointment using Homecare Homebase (HCHB); OASIS proficiency is required for RNs.
  • Attend periodic office meetings at the assigned clinic location.
  • May be asked to float to a nearby clinic to pick up additional patients if local census is low.
Required Qualifications
  • Active, unrestricted RN license in hand - pending licenses are not accepted.
  • Minimum of one full year of recent adult home health experience.
  • First-time travelers are not accepted; recent specialty experience is required.
  • CPR and BLS certifications required.
  • Proficiency in Homecare Homebase (HCHB) used within the past 6 months; clinicians outside this window may be considered but must complete retraining during compliance.
  • OASIS proficiency required and used within the past 6 months; a retraining course may be completed during compliance if outside this window.
  • Reliable personal vehicle, valid driver's license, and agency-required auto liability insurance required; mileage between patient homes is reimbursed at the IRS standard rate.
  • Must be willing to travel up to 75 miles from the assigned care center, which may result in drives longer than 20 minutes between patients.
  • All documentation must be completed within 24 hours of each patient appointment; delayed or incomplete documentation may result in contract cancellation.
  • Must be open to on-call rotations; weekend and on-call requirements vary by location and will be discussed during the interview.
  • Local candidates are welcome and accepted at the same rate.
Preferred Experience / Skills
  • Experience with Start of Care assessments and wound care in a home health setting.
  • Familiarity with adult home health case management and productivity-based scheduling models.
  • Prior experience navigating multi-patient daily schedules across a broad geographic territory.
