Key Responsibilities

• Conduct 6-8 skilled home visits per day for patients under Medicare, Medicare Advantage, and commercial insurance plans
• Perform cardiopulmonary assessments, wound care (including ABI compression dressings and wound vac dressings), Pleurx/Aseptdrain care, foley catheter changes, and staple/suture removal
• Administer IV therapy, manage PICC line and Midline care and dressing changes, and perform SQ port access and deaccess
• Conduct lab draws via venipuncture, SQ/IM injections, and utilize glucometer and pro-time machines
• Complete OASIS documentation and self-schedule visits based on the needs of the home care office
• Work a variable schedule that may include Monday-Friday day shifts, Thursday-Monday or Friday-Tuesday rotations, and alternating weekend shifts

Required Qualifications

• Active Florida RN license or valid Compact RN license required at time of submission
• Minimum 1 year of skilled home health experience (private duty nursing experience alone will not be accepted)
• Infusion experience required; strong IV skills and wound care proficiency required
• OASIS training and experience required
• Basic Cardiac Life Support (BCLS) certification required
• Valid driver's license and personal vehicle required to conduct home visits
• Must be COVID-19 vaccinated or have an exemption form on file; flu shot or declination required (mandatory mask wearing applies in applicable cases)
• Candidates must reside more than 100 miles from the facility to be eligible

Preferred Experience / Skills

• Cerner EMR experience preferred
• Experience managing high-risk diagnoses including Heart Failure, COPD, MI, Pneumonia, and Stroke
• Experience with post-operative care and general medical diagnoses
• Comfort with self-scheduling and working independently in a field-based environment
