This nurse provides safe, competent quality care based on nursing theory and research. Care is delivered to a designated group of patients in their home setting. Patient care incorporates the tenets of the professional practice model of Relationship Centered Care. The nursing process is applied based on theory, research, evidence-based practice, and approved organizational standards. Patient care needs are assessed and evaluated utilizing critical thinking skills. Relevant assessment and intervention skills are integrated into the delivery of care. Collaboration with the patient, prescribing provider, and interdisciplinary team members is essential. The development, implementation, and evaluation of the plan of care take place in this partnership. Nursing care is provided per provider orders, which includes establishing peripheral intravenous access. Intravenous therapy, laboratory sample collection, and wound care are included in the services offered. Individualized, measurable goals are established in consultation with patients and their families. Regular evaluation of progress toward the established goals is conducted. Prioritization of patient care aspects, including education and planning for post-discharge care, is vital. Effective communication with patients, families, and all members of the interdisciplinary team is required. Cultural diversity and expressed needs are considered in all care aspects. A therapeutic relationship with patients and families is established and maintained. Evidence-based practices are used to enhance understanding of patients' perceptions of care. Service recovery is initiated for patients and family concerns and escalated appropriately. Demonstrating caring, respect, compassion, empathy, and active listening is essential. Safe and efficient patient-centered care practices are implemented. Clinical skills and knowledge crucial to quality and safety in the patient population served should be demonstrated. Process improvement strategies based on institutional evidence and practices are implemented. Engagement in peer and institutional review processes is encouraged. Collaboration with interdisciplinary teams to create and implement quality improvement projects is a responsibility. Contributions to quality and safety practices and compliance measures are expected. The nurse manages a designated caseload of patients to ensure progress toward care goals. Services provided must align with treatment orders and the plan of care.
