Registered Nurse - Care Manager
Job Summary
We are seeking a dedicated and skilled Registered Nurse with a specialization in Care Management to join our healthcare team in Wrangell, Alaska. This role involves providing comprehensive professional nursing care across the lifespan, utilizing the nursing process to assess, plan, implement, and evaluate patient care. The successful candidate will deliver high-quality, culturally sensitive, and patient-centered services within a collaborative interdisciplinary environment.
As a Care Manager, you will play a crucial role in coordinating patient care, facilitating referrals, and ensuring continuity of services. You will advocate for patients, assist them in navigating the healthcare system, and collaborate closely with providers and other team members to promote optimal patient outcomes, safety, and a positive patient experience.
Key Responsibilities
- Perform comprehensive patient assessments and develop individualized, evidence-based plans of care.
- Implement and evaluate nursing interventions, recognizing changes in patient condition and initiating appropriate responses or escalation of care.
- Administer medications and treatments safely and accurately according to provider orders and organizational policies.
- Provide thorough patient and family education regarding care, medications, and discharge planning.
- Exercise independent nursing judgment and critical thinking to determine appropriate nursing actions, interventions, and priorities, consistent with the Alaska Board of Nursing scope of practice.
- Collaborate effectively with providers and interdisciplinary team members to coordinate care, facilitate referrals, and ensure continuity of services.
- Document assessments, interventions, and patient outcomes accurately and timely in the electronic health record (EHR).
- Advocate for patient care needs and assist patients in navigating the healthcare system.
- Adhere to nursing scope of practice, regulatory requirements, safety protocols, and infection prevention standards.
- Participate in quality and performance improvement activities.
- Demonstrate professionalism, accountability, flexibility, and teamwork within the clinical setting.
- Provide appropriate guidance and delegation to ancillary staff.
- Support the orientation, mentoring, and education of staff, students, and peers.
- Float to other departments or assignments as needed to support patient care needs.
Required Qualifications
- Associate’s Degree in Nursing (ADN) from an accredited program.
- Basic Life Support (BLS) certification.
- Clinical competency validation within 3 months of hire and every 3 years thereafter.
- High-risk competency validation within 3 months of hire and annually thereafter.
- For new nursing program graduates or experienced nurses new to a specialty area: All required specialty-specific and advanced certifications (e.g., ACLS, PALS, NRP, TNCC, ENPC, as applicable to practice area) must be obtained within six (6) months of hire.
- For experienced nurses hired directly into a specialty area: Evidence of current required certifications for the position (e.g., ACLS, PALS, NRP, TNCC, ENPC, as applicable to practice area) at the time of hire.
- For nurses with float areas: Completion of required certifications for the highest level specialty in which care will be provided.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Case Management certification.
