Job Summary

This is a vital role focused on managing the care pathways for OB patients, ensuring continuity, advocacy, and support.

Key Responsibilities
  • Provide comprehensive, patient-centered nursing care to obstetric patients in an outpatient setting.
  • Utilize the nursing process (assessment, diagnosis, planning, implementation, evaluation) to develop individualized care plans.
  • Administer medications and treatments safely and accurately according to provider orders and organizational policy.
  • Perform clinical procedures and recognize changes in patient condition, initiating appropriate interventions or escalation of care.
  • Deliver thorough patient and family education regarding care plans, medications, and discharge needs.
  • Coordinate care for OB patients, facilitating referrals, follow-up appointments, and continuity of services.
  • Document patient assessments, interventions, and outcomes accurately and timely in the electronic health record (EHR).
  • Collaborate effectively with providers, interdisciplinary teams, and community resources to optimize patient outcomes.
  • Adhere to organizational standards, policies, evidence-based practice, infection control, and regulatory requirements.
  • Participate in quality improvement activities and maintain a clean, safe working environment.
  • Advocate for patient needs and assist patients in navigating the healthcare system.
Required Qualifications
  • Minimum of one (1) year of recent experience in OB Case Management.
  • Graduate of an accredited School of Nursing.
  • High School Diploma or equivalent.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Case Management Certification (e.g., CCM).

  • Clinical Expertise: Proficient in comprehensive patient assessment, clinical decision-making, medication administration, and recognizing/responding to changes in patient status.
