Registered Nurse Case Manager
Job Summary
We are seeking an experienced Registered Nurse Case Manager for a 13-week contract assignment in an acute care hospital setting in Bozeman, Montana. This role focuses on coordinating care for adult Med-surg patients.
The Case Manager will collaborate with physicians and a multidisciplinary team to develop, implement, and monitor comprehensive plans of care. Key responsibilities include ensuring patient progression towards desired outcomes, identifying and resolving barriers to effective care, and actively participating in the discharge planning process.
Key Responsibilities
- Complete comprehensive patient assessments within regulated timelines to determine eligibility for case management services.
- Develop and implement individualized case management plans in collaboration with patients, caregivers, physicians, and other healthcare professionals.
- Perform ongoing monitoring and evaluation of care plans, documenting interventions, tracking goal achievement, and recommending adjustments as needed.
- Manage an ongoing patient caseload, providing regular outreach and support.
- Promote the integration of services, including behavioral health and long-term care, to enhance continuity of care for patients.
Required Qualifications
- Minimum of two (2) years of nursing experience.
- Demonstrated experience in Utilization Review.
Preferred Qualifications
- Bachelor’s Degree in Nursing (BSN).
- Two (2) years of acute care hospital experience as a Case Manager.

Shift Details

- Potential for work beyond standard 40-hour week, including weekends and after-hours shifts.
- On-call work may be required.
Licensing Requirements
- Active Registered Nurse (RN) license in the State of Montana.
- Current Basic Life Support (BLS) certification from the American Heart Association.
