Job Summary
We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as an Outpatient Case Manager in Glenwood Springs, Colorado. This vital role focuses on facilitating seamless patient transitions and ensuring optimal access to healthcare services within an outpatient setting, often collaborating with inpatient teams.
The successful candidate will be instrumental in coordinating the overall admission process for both inpatients and outpatients, guiding them through their healthcare journey. This position requires a proactive approach to patient advocacy and a strong understanding of healthcare navigation to ensure patients receive timely and appropriate care.
Key Responsibilities
- Coordinate the admission process for both inpatient and outpatient populations.

Required Qualifications
- Graduation from an accredited or state board of nursing approved Registered/Professional Nursing program.
- Minimum of three (3) years of experience as an RN Case Manager.
