Job Summary

We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as an Outpatient Case Manager in Glenwood Springs, Colorado. This vital role focuses on facilitating seamless patient transitions and ensuring optimal access to healthcare services within an outpatient setting, often collaborating with inpatient teams.

The successful candidate will be instrumental in coordinating the overall admission process for both inpatients and outpatients, guiding them through their healthcare journey.
