Registered Nurse - Skilled Nursing Facility / Long-Term Care
Job Summary
We are seeking a dedicated and experienced Registered Nurse (RN) to join our team in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) unit located in Harvard, Illinois. This role offers an opportunity to provide comprehensive nursing care to residents requiring skilled nursing, rehabilitation, and long-term support.
The successful candidate will be instrumental in delivering high-quality, compassionate care, focusing on patient advocacy, safety, and positive outcomes. You will work collaboratively within an interdisciplinary team to ensure residents receive individualized and holistic care.
This position involves applying the full scope of the nursing process, from assessment and care planning to intervention and evaluation, while maintaining accurate documentation and engaging in continuous professional development.
Key Responsibilities
- Apply the nursing process (assessment, planning, intervention, evaluation) to deliver comprehensive patient care in a SNF/LTC setting.
- Administer medications and treatments safely and accurately in accordance with established policies, procedures, and state regulations.
- Develop, implement, and evaluate individualized patient care plans based on patient needs and physician orders.
- Maintain accurate, thorough, and timely documentation of patient status, interventions, and outcomes.
- Provide patient and family education tailored to their level of understanding and support their care journey.
- Employ critical thinking to ensure patient safety, manage complex clinical situations, and delegate tasks appropriately to other staff.
- Actively participate in quality improvement initiatives, performance improvement activities, and shared governance.
- Provide clinical guidance and support to interdisciplinary team members, fostering a collaborative care environment.
- Engage in continuous professional development and support evidence-based practice guidelines.
- Participate in peer review and provide constructive feedback to colleagues.
Required Qualifications
- Graduate of an accredited nursing program.
- Prior experience as a Registered Nurse in a Skilled Nursing Facility or Long-Term Care setting.
