We are seeking a dedicated and experienced Registered Nurse (RN) to join our team in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) unit located in Harvard, Illinois. This role offers an opportunity to provide comprehensive nursing care to residents requiring skilled nursing, rehabilitation, and long-term support.

Key Responsibilities
  • Apply the nursing process (assessment, planning, intervention, evaluation) to deliver comprehensive patient care in a SNF/LTC setting.
  • Administer medications and treatments safely and accurately in accordance with established policies, procedures, and state regulations.
  • Develop, implement, and evaluate individualized patient care plans based on patient needs and physician orders.
  • Maintain accurate, thorough, and timely documentation of patient status, interventions, and outcomes.
  • Provide patient and family education tailored to their level of understanding and support their care journey.
  • Employ critical thinking to ensure patient safety, manage complex clinical situations, and delegate tasks appropriately to other staff.
