Job Responsibilities: Performs assessments and reassessments per policy and procedure or as warranted by patient condition. Demonstrates the ability to make sound independent decisions. Initiates, prioritizes, and implements the plan of care. Reviews and revises the plan as indicated. Collaborates with patient/family and interdisciplinary services. Utilizes resources appropriately to support the needs of the organization, patient, and family. Monitors and documents responses to clinical interventions. Facilitates, coordinates, provides, and evaluates education specific to patient/family needs. Demonstrates and encourages teamwork. Identifies and appropriately communicates opportunities for improvement. Participates in quality, safety, and service process improvement efforts.
