Responsibilities include: - Performing and documenting comprehensive patient assessments in accordance with the RN’s scope of practice and hospital policies. - Considering the patient’s culture, age, and developmental level. - Identifying patient risk factors and presenting complaints. - Collecting both objective and subjective data. - Verifying admission assessments when obtained by others. - Developing a prioritized problem list based on input from the interdisciplinary team. - Designing actions and interventions relevant to the patient’s culture and age. - Determining desired outcomes in coordination with the patient and family in accordance with the medical plan of care. - Implementing patient care therapies and procedures within the established policies and cultural competencies. - Explaining procedures, treatments, and processes to patients and families. - Initiating actions to alleviate patient stress and discomfort. - Recognizing, responding to, and communicating changes in patient conditions appropriately. - Conducting ongoing assessments and documenting responses to therapies in accordance with hospital policies. - Delegating tasks appropriately and overseeing team members in patient care activities. - Monitoring the accuracy and completeness of delegated patient care. - Identifying and addressing educational needs of patients related to prioritized problems. - Facilitating the patient discharge process by executing discharge orders in a timely manner. - Collaborating with Case Management and others as indicated.
