Registered Nurse (RN) Case Manager
Job Summary
We are seeking an experienced and dedicated Registered Nurse (RN) Case Manager for an 8-week contract assignment in Grand Junction, Colorado. This role is integral to the ongoing development, implementation, and evaluation of our Case Management Plan within a hospital setting. The RN Case Manager will work collaboratively with an interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate patient care options and services.
The successful candidate will serve as a patient advocate, facilitating communication, managing resources effectively, and providing education to ensure high-quality, cost-efficient patient care. This position requires a strong understanding of regulatory compliance, utilization review processes, and the ability to optimize hospital resources while adhering to Federal and State regulations related to billing and fiscal intermediaries.
Key Responsibilities
- Maintain and improve the case management system through ongoing interaction with patients, physicians, and other healthcare providers to achieve designated clinical, operational, and financial outcomes for aggregate patient populations.
- Assist physicians, patients, families, staff, and payers in providing appropriate, effective, and cost-efficient care.
- Collaborate with payers and review organizations to ensure proper utilization of patient care resources.
- Educate physicians and staff regarding appropriate levels of care and utilization issues.
- Document clearly and concisely all contacts and information pertinent to the patient's case management process.
- Attend and actively participate in all departmental and interdepartmental meetings related to Case Management and Social Services functions.
- Maintain current knowledge of Case Management, Social Services, discharge planning, and nursing issues through continuing education.
- Participate in professional activities to foster continuous professional development and contribute to the profession.
Required Qualifications
- Bachelor's Degree in Nursing.
- Minimum of three (3) years of clinical experience in a hospital setting.
- Minimum of five (5) years of case management experience in a hospital setting.
Preferred Qualifications
- Master's Degree in Nursing.
- Certification in Case Management (CCM) or willingness to obtain within two (2) years of employment.
Skills & Competencies
- Strong knowledge of the continuum of care, discharge planning, and utilization review processes.
- Excellent documentation skills, with the ability to create clear and concise Case Management plans.
- Exceptional communication, collaboration, and advocacy skills.

- Weekend Coverage: Required for 5 hours on one Saturday per month, with 5 hours of compensatory time off during one weekday.

Licensing Requirements
- Current, unrestricted Registered Nurse (RN) license in the State of Colorado.
- Current Basic Life Support (BLS) certification.
