**Job Description:**

**What You'll Do:**
- Collaborate with a multidisciplinary diabetes management team to educate patients and families living with diabetes mellitus.
- Utilize the nursing process and work closely with interdisciplinary team members to deliver exceptional care.
- Administer prescribed medications and treatments following approved techniques, policies, and state regulations.
- Document patient care accurately according to established standards.
- Develop, initiate, and execute personalized patient care plans, including thorough instruction for patients and families.
- Participate in performance improvement activities to enhance department or unit quality.
- Offer clinical expertise and guidance to support the patient's longitudinal care plan.
- Employ critical thinking to ensure patient safety and elevate the patient experience.
- Serve as a resource to staff on patient education programs and materials development.
- Advocate for patients while addressing clinical, psychosocial, and financial issues.
**Position Requirements:**
- Graduate of an accredited nursing program (Education Verification required).
- BSN preferred.
- Licensed as an RN in IL and WI.
- Approved CPR certification required within 90 days.
- CDCES certification required.
- Minimum of 2 years of experience.
**Shift Information:**
- Monday to Friday Day Shift Hours.
