Description

American Traveler is hiring an RN Case Manager for an inpatient Care Management role requiring CA RN licensure and RN case management certification.

Details

  • Unit: RevCycle - Care Management within an acute care hospital setting
  • Day shift, 8:00 AM-4:30 PM, Sunday-Thursday, 40 hours per week
  • No on-call requirements
  • Handles a heavy caseload including complex patients, frequent ED utilizers, chronic pain, substance abuse, and homeless patient populations
  • Coordinates continuum of care from pre-admission through post discharge
  • Collaborates with acute care case managers and coordinates transfers to tertiary centers

Requirements

  • Active CA RN license required
  • RN Case Management (CM) certification required
  • BLS certification required
  • Inpatient case management experience required
  • Experience with strike force staffing models (KP or HCA CA) preferred
  • BSN and prior case management role experience preferred
  • Minimum 2 years of experience in specialty, with recent experience in the last 3 months
  • Cerner and Epic experience must be noted in employment history if applicable

Additional Information

  • Acts as liaison between patient, family, physicians, multidisciplinary team, and healthcare benefactors
  • Monitors appropriateness and timeliness of interdisciplinary care plans and resource utilization
  • Provides oversight and data collection for regulatory and accreditation compliance
  • Conducts focus study reviews for frequent ED visitors
  • Candidates with more than 5 days of requested RTO will be automatically declined
  • Candidates with any prior adverse licensure action will not be considered
  • No radius rule in place; local candidates accepted at the same rate as travel candidates
  • Travelers may only be submitted to one hospital within the Adventist Health system at a time
