Job Title

Contract | Full-Time | Days | Monday-Friday, 7:00 AM - 3:30 PM | No weekends, holidays, or on-call
Key Responsibilities

• Manage a caseload of approximately 8-10 patients per day across inpatient, observation, and swing bed settings
• Perform utilization review (UR), including Notice of Admission (NOA) reviews, and discharge planning
• Navigate insurance portals to support authorization and UR processes
• Coordinate discharge planning with pharmacy and other interdisciplinary team members
• Work with common patient populations including COPD, CHF, and debility (swing bed patients)
• Document and manage cases using the Meditech EMR system

Required Qualifications

• Active RN (Registered Nurse) license
• Minimum of 2 full years of case management experience required
• BLS certification required
• Demonstrated experience with insurance portal navigation
• This is a non-clinical role - no direct clinical patient care involvement

Preferred Experience / Skills

• CCM (Certified Case Manager) certification preferred but not required
• Experience with Meditech EMR
• Background in utilization review and discharge planning within an inpatient or hospital setting
