Contract | Days | 5×8-hour shifts, 8:00 AM - 4:30 PM | 2 days onsite, 3 days remote per week
Key Responsibilities

• Manage complex cases across the health system and support patients in the community through virtual care delivery
• Float between office locations in the San Francisco and Emeryville areas as needed
• Coordinate care plans and resources for patients with complex medical and social needs
• Collaborate with interdisciplinary teams to facilitate appropriate transitions of care
• Conduct virtual case management activities for community-based patients

Required Qualifications

• Active California RN License (must be verifiable via Nursys, uploaded within the last 2 weeks)
• Minimum 2 years of inpatient case management experience
• All applicable certifications must be current and from AHA or ARC
• 2 professional references obtained within the last year required
• 7-year work history verification required
• Local candidates are welcome; must reside within 50 miles of the facility to be eligible for local rate

Preferred Experience / Skills

• Experience with community-based or virtual case management
• Familiarity with complex discharge planning and care coordination across care settings
• Ability to adapt to a hybrid work environment with flexibility to float between office locations
