Key Responsibilities

• Provide case management services within an acute care hospital setting
• Coordinate patient care planning, discharge planning, and utilization review
• Float between like units based on facility need
• Document and chart patient information using Epic EMR
• Participate in every-other-weekend (EOW) scheduling rotation
• Commit to working 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Required Qualifications

• Active RN license required; compact license must be in hand at time of submission
• Minimum of 2 years of case management experience; at least 1 year required - first-time travelers considered with clinical approval
• Acute care experience required
• Must submit all requested time off (RTO) at the time of submittal; post-offer RTO may not be considered
• 2 professional references covering 1 year of experience within the last 3 years required prior to start
• Local candidates are welcome; candidates living within 50 miles of the facility may be accepted at a different rate

Preferred Experience / Skills

• Prior experience with Epic EMR charting system
• Experience floating between units or sister facilities
• Strong communication and care coordination skills in a fast-paced acute care environment
