Schedule / Job TypeContract (Temp-to-Hire); Monday - Friday, shift start ranging from 8:00 AM - 9:00 AM for 8.5 hours; weekend rotation required once every four weeks
Key Responsibilities- Provide case management services within an acute care hospital setting
- Coordinate patient care plans, discharge planning, and utilization review
- Collaborate with interdisciplinary teams to facilitate appropriate and timely patient transitions
- Monitor and manage patient progress to ensure quality outcomes and efficient use of resources
Required Qualifications- Active Pennsylvania RN license required
- BSN required
- Minimum 2 years of acute care Case Manager experience required; non-acute backgrounds will not be considered
Preferred Experience / Skills- Experience with utilization management and discharge planning in a hospital environment
- Candidates open to permanent placement upon contract completion are highly preferred
