We are seeking an experienced Case Management Registered Nurse (RN) to join an outpatient clinic in Sitka, AK. In this role, you will coordinate comprehensive patient care plans, facilitate safe and timely discharges, and serve as a key liaison between patients, families, and the multidisciplinary care team. Key Responsibilities
• Assess patients' medical, psychosocial, and functional needs to develop individualized care coordination plans
• Collaborate with physicians, social workers, and interdisciplinary teams to facilitate appropriate level-of-care transitions
• Coordinate timely discharge planning to ensure safe transitions to post-acute or community-based settings
• Identify and address barriers to discharge, including social determinants of health and insurance authorization needs
• Conduct utilization review and monitor patient status to support appropriate admission criteria and length of stay
• Educate patients and families on discharge instructions, community resources, and follow-up care plans
• Document all case management activities accurately in the electronic health record in accordance with regulatory standards
✅ Requirements
• Active AK or Compact RN license
• BLS, Case Management Certification (preferred) certifications
• 1+ years of Case Management experience
• Case Management certification (ACM or CCM) preferred
Pay
Salary range: $86,000 to $117,000 per year.
