**Key Qualifications:**
- BSN required; Master's degree in nursing, Health Administration, or Business Administration preferred.
- Case Management certification (e.g., CCM or ACM) preferred.
- Minimum of five (5) years of experience in an acute healthcare setting required.
- Minimum of five (5) years of progressively responsible leadership/administrative experience in:
- Case Management
- Care Coordination
- Utilization Management
- Related functions
- Demonstrated experience in:
- Leading interdisciplinary clinical/operational programs
- Staff development
- Performance improvement
- Regulatory or accreditation readiness
- Advanced knowledge of:
- Case management
- Care coordination
- Utilization management
- Discharge planning
- Patient progression
- Transitions of care in an acute-care environment
**Shift Information:**
- Day Shift: Monday - Friday
**Certifications / Requirements:**
- BLS/BCLS certification
