**Exciting Opportunity for Experienced Registered Nurse Case Manager!**
**Job Description:**
- Join a dynamic team as an Ambulatory Registered Nurse (RN) Case Manager in a fast-paced outpatient clinic environment.
- Provide comprehensive, patient-centered nursing care supporting individuals and families across the continuum of care.
- Utilize the nursing process to:
- Assess, plan, implement, and evaluate nursing care.
- Administer medications and treatments safely and accurately.
- Perform clinical procedures and provide patient education.
- Ensure accurate documentation and adherence to infection control and regulatory standards.
- Promote quality outcomes, patient safety, and culturally responsive care.
**Key Responsibilities:**
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Implement and evaluate nursing interventions and patient responses.
- Recognize changes in patient condition and initiate appropriate interventions.
- Deliver culturally responsive care tailored to each patient's needs.
- Collaborate with providers and interdisciplinary teams for seamless care coordination.
- Document assessments, interventions, and outcomes accurately in the electronic health record.
**Required Qualifications:**
- Active Registered Nursing license in Alaska or other U.S. state.
- Case Management experience in a clinic or hospital setting within the last 2 years.
- Graduate of an accredited school of nursing; Bachelor's degree preferred.
- Basic Life Support (BLS) certification.
- Strong nursing judgment and critical thinking skills.
