Job Title: Ambulatory Care Registered Nurse Case Manager Profession: Nursing Specialty: Care Manager Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8 Experience: Minimum 1 year of case management experience in a clinic or hospital setting within the last 2 years License: Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required Certifications: Basic Life Support certification required Must-Have: Active Alaska license at time of submission Description: The Ambulatory Registered Nurse Case Manager provides comprehensive, patient-centered nursing care in an outpatient clinic setting. This role supports individuals and families across the continuum of care. The Ambulatory RN utilizes the nursing process to assess, plan, implement, and evaluate care. Responsibilities include administering medications and treatments, performing clinical procedures, and providing patient education and care coordination. Collaboration with providers and interdisciplinary teams is essential. The Ambulatory RN ensures accurate documentation and adherence to infection control and regulatory standards. Efficient clinic operations are crucial while promoting quality outcomes and patient safety. Culturally responsive care is a priority in a fast-paced ambulatory environment. Patient Care: Performs comprehensive patient assessments and develops individualized plans of care. Implements and evaluates nursing interventions and patient responses. Administers medications and treatments safely and accurately per provider orders and policy. Recognizes changes in patient condition and initiates appropriate interventions or escalation of care. Provides patient and family education related to care, medications, and discharge needs. Delivers culturally responsive, patient-centered care. The Registered Nurse independently applies critical thinking and professional judgment. Performs and interprets comprehensive assessments to recognize significant changes in patient condition. Exercises independent nursing judgment in determining appropriate actions, interventions, and priorities. Identifies when consultation, collaboration, or escalation to a licensed independent provider is required. Develops, implements, evaluates, and revises individualized nursing plans of care based on patient needs. Integrates clinical findings with ethical principles, cultural considerations, patient safety standards, and regulatory requirements. Accepts accountability for nursing decisions, delegated tasks, and outcomes of care in accordance with statutes and regulations. Care Coordination and Documentation: Collaborates with providers and interdisciplinary team members to coordinate care. Facilitates referrals, follow-up care, and continuity of services. Documents assessments, interventions, and outcomes accurately and timely in the electronic health record. Advocates for patient care and assists patients in navigating the healthcare system.
