Must-Have:
Prior recent experience in care coordination in an emergency department.
Familiarity with Interqual criteria and utilization review.
Description:
This position involves case management and care coordination.
You will be responsible for managing both discharge planning and utilization management.
Daily responsibilities include dealing with all payers, including uninsured and homeless patients.
You will complete initial assessments and discharge planning.
Collaboration and teamwork with various departments are essential.
Effective communication skills are necessary to adapt throughout the day.
The case manager will handle approximately 15-20 patient consultations per day.
Training in Interqual criteria will be provided.
Weekend work is required on an every other basis, with a minimum of one full weekend per month.
While holiday work is not required, willingness to work on holidays is appreciated.
This position may require coverage across various areas of the facility.
The work setting is primarily acute care.
Reprioritization of tasks throughout the day is expected.
The charting system used is Cerner.
