Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department. Strives to promote patient wellness and improve care outcomes. Ensures efficient utilization of health services and minimizes denials of payment among a patient population with complex health needs. Requires experience in acute hospital and long-term acute care settings. Familiarity with medical necessity, retrospective review, and concurrent review is essential.
