Job Title: Case Manager
Location: Eureka, CA 95501
Duration: 26 Weeks
Start Date: November 16, 2026
Shift: Days | 5×8 Hours | 8:00 AM – 4:30 PM
Job Requirements:
Minimum 2 years of acute hospital Case Management experience.
Experience in care coordination and discharge planning required.
Epic charting experience required.
Travel experience required; first-time travelers may be screened with clinical approval.
Must be willing to work every other weekend.
Floating between similar units may be required based on facility needs.
Local candidates accepted.
No on-call requirements.
All requested time off (RTO) must be submitted at the time of application.
Key Responsibilities:
Case management and utilization review.
Care coordination, discharge planning, and needs assessment.
Admission criteria, appeals and denials, and benefits eligibility.
DME coordination and prior authorizations.
Additional Information:
Free parking on the first day; ongoing parking details provided during orientation.
