Travel Nurse RN - Case Manager - $2,900 to $3,000 per week in Orange, CA

We are seeking a dedicated and experienced Registered Nurse (RN) – Case Manager to provide comprehensive care coordination, particularly for patients with neuro-oncology needs. The RN Case Manager will play a key role in developing and managing individualized care plans, ensuring seamless coordination across services and providers.

• Develop and manage customized patient care roadmaps
• Coordinate follow-up appointments and ongoing care services
• Order and track labs, imaging, and diagnostic tests
• Facilitate referrals and obtain necessary authorizations
• Coordinate Durable Medical Equipment (DME) orders and delivery
• Provide end-to-end care coordination across multidisciplinary teams
• Lead transition planning (e.g., hospital to home or other care settings)
• Conduct patient needs assessments and adjust care plans accordingly
• Work closely with neuro-oncology patients and care teams
