Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 10 hours Experience: Minimum 2 years of Case Management experience with strong acute care experience and critical care experience. License: Valid state license required. Certifications: BCLS, BSN Must-Have: Strong EPIC experience, history of travel assignments, and experience arranging post-acute services. Description: The RN Care Coordinator is responsible for determining the appropriateness of hospital admission. The role involves advocating, coordinating, and facilitating the interdisciplinary plan of care. This is done to expedite the medically appropriate and effective utilization of resources for maximum patient outcomes. Strong discharge assessment and discharge planning skills are required. Experience in arranging for post-acute services such as Home Health, Home Infusion, and Skilled Nursing is essential. Additional responsibilities include managing DME, hospice services, dialysis, and outpatient rehab. The candidate will also be involved in discharge appeals and logistical needs for patient transitions. Travelers must have experience with Epic Charting. Note that this position requires working 2 weekend shifts per 4-week period.
