Must-Have:
- Experience in Care Coordination, Care Transitions, and Discharge Planning.

- Ability to coordinate with a multidisciplinary team to ensure discharge is on track.
Description:
The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the emergency department visit.
They will complete timely documentation and discharge planning.
They will handle referrals to post-acute levels of care and transport.
The role involves coordinating with the multidisciplinary team to ensure that patient discharges are on track.
Experience with admission criteria, appeals and denials, and medical necessity is required.
Candidates must possess knowledge of HIPAA guidelines and prior authorizations.
The RN Case Manager will regularly work with various patient populations, primarily adults and geriatrics.
Maintaining compliance with regulations and effective communication with healthcare teams are essential parts of this position.
