Key Responsibilities of the RN - Case Management- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for individualized care plans
- Assess cost-efficient levels of care and put non-medical support in place
- Coordinate care transitions and engage specialty resources
- Maintain detailed records of outcomes during the management process
- Identify opportunities for health promotion and illness prevention
- Act as patient advocate by protecting privacy and confidentiality
Requirements of the RN - Case Management- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted WV or compact Registered Nurse license
- Ability to analyze complex medical information and make informed decisions
- Case Management certification
