This role is essential for managing patient care coordination and discharge planning. The ideal candidate will have experience navigating admission criteria and benefits eligibility. Experience in acute hospital settings and care coordination is needed. The candidate should demonstrate proficiency in working with diverse patient age groups, including neonates, adolescents, adults, and geriatrics. Experience with telephonic case management and knowledge of DRG is advantageous. We prefer a candidate with experience in community hospital settings and working with patients in various healthcare settings.
