**Join Our Team as a Discharge Planner!**
**Job Description:**
- Exciting opportunity for a Discharge Planner to coordinate the discharge planning care process for in-patients.
- Work with a diverse patient population, including those with chronic diseases and special health care needs.
- Engage in case management within Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit.
- Maintain a comprehensive tracking system for referred patients to various healthcare facilities.
- Perform thorough chart reviews for requested services to ensure smooth transitions, whether returning to the facility or discharging home.
**Schedule:**
- Monday to Friday
- 8:00 a.m. to 5:00 p.m. (includes one-hour lunch)
- Attend staff meetings as required
**Requirements:**
- Minimum of 2 years of experience in discharge planning and Med-Surg
- RN experience in clinical, medical/surgical, acute, or critical care settings
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory
- Valid American Heart Association (AHA) certification in Basic Life Support (BLS)
