Must-Have: - Experience with assessing patients, care coordination, and discharge planning. - Ability to organize and participate in family meetings. - Knowledge of care management processes. - Competent with arranging durable medical equipment and transport services. - Familiarity with electronic charting systems is required. Description: This position requires case managers to coordinate patient care effectively. Floating to different facilities may be required. The nurse will be responsible for managing a patient ratio of 1:25. Days off will vary and are not guaranteed to be back-to-back, although scheduling efforts will be made to accommodate consecutive days off when possible. The position will involve working in a fast-paced environment where teamwork is essential. Candidates will engage in the development of treatment plans and contribute to interdisciplinary care approaches. Experience with various patient populations, including those with mental health issues and substance abuse histories, is beneficial.
