License: California state license required; pending license accepted but must be licensed upon start date. Certifications: Current certifications related to nursing and care management. Must-Have: - Experience in acute care hospital settings is essential. - Knowledge of drug and alcohol abuse treatment options. - Ability to assess patients and develop treatment plans. - Skills in care coordination and discharge planning. - Competency in arranging durable medical equipment, skilled nursing facilities, and transportation independently. Description: The Case Manager role involves providing care coordination and discharge planning for patients. Registered Nurses will work with a patient ratio of 1:20.
