Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 26 Weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years acute medical Care Manager experience performing Care Coordination and Discharge Planning License: California state license required; pending license accepted, but must be licensed upon start date Certifications: Relevant nursing certifications required Must-Have: Experience in care coordination, discharge planning, and computerized documentation Description: This position is for a Case Manager in an acute care facility. The expected patient ratio during day shifts is 1:25. Weekend work is typically required for one full weekend every four weeks, subject to departmental needs. Experience with health maintenance organizations, independent practice associations, and assessing patients is essential. Candidates must have experience with care coordination and discharge planning. Familiarity with the Centers for Medicare and Medicaid Services guidelines is preferred. Knowledge of current procedural terminology coding and billing is a plus. Providers may be required to float to nearby facilities within the system. Travelers must be prepared to float as needed; refusal to do so may result in termination of assignment. Candidates should be prepared to work during holidays, with at least three of the five major holidays required. Providers will be supplied with necessary communication devices for their duties at no cost. Submissions must include responses to questions regarding previous employment experiences related to similar roles. References covering at least one year within the last three years are required. Travelers may experience variations in workload and should adapt accordingly. This role requires adherence to HIPAA guidelines and knowledge of medical necessity determination per evidence-based guidelines. Candidates should possess strong organizational and interpersonal skills for effective patient care management.
