**Exciting Opportunity for RN Case Manager**
**Job Description:**
- Join a dynamic team focused on optimizing patient care and resource utilization!
- Coordinate management of care to ensure exceptional service delivery and compliance with external review agencies.
- Provide ongoing support and expertise through comprehensive patient assessments and care planning.
- Enhance patient management quality and satisfaction promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions for holistic patient care.
- Advocate for patients by addressing their resource needs and preferences throughout the care process.
- Collaborate with interdisciplinary teams (IDT) in long-term acute care (LTAC) or short-term acute care hospital (STACH) settings.
**Facility Requirements:**
- BLS certification must be attached to your submission packet (from the American Heart Association).
- Minimum of 2 years recent RN Case Management experience.
- Experience with MCG and discharge planning across acute care settings is essential.
- Current California RN license must be attached to your resume.
- Monday to Friday shift: 8:00 AM - 4:30 PM.
