**Job Description:**

- Coordinate management of care, ensuring excellence in service delivery and compliance with external review agencies.
- Provide ongoing support and expertise through:
- Comprehensive assessment
- Care planning
- Implementation of care plans
- Overall evaluation of individual patient needs
- Enhance patient management quality and satisfaction.
- Promote continuity of care and cost-effectiveness by integrating:
- Case management
- Utilization review
- Discharge planning
- Advocate for patients by:
- Utilizing resources effectively
- Addressing holistic needs
- Ensuring patient preferences are prioritized.
**Facility Requirements:**
- BLS certification required (must be attached to submission packet, and through the American Heart Association).
- Minimum 2 years of recent RN Case Management experience.
- Experience in LTAC or STACH settings is preferred.
- Familiarity with MCG, discharge planning, and strong interdisciplinary team (IDT) collaboration is a must.
- California RN license required (must be attached to resume).
- Work Schedule: Monday - Friday, 7:30 AM - 4:00 PM.
