**Exciting Opportunity for RN Case Manager - Travel Position!**
Join our dynamic team as an RN Case Manager, where your skills will be instrumental in enhancing patient care and satisfaction!
**Key Responsibilities:**
- Coordinate and facilitate patient care through collaboration with the Interdisciplinary Care Transitions (ICT) team.
- Follow patients throughout their continuum of care, ensuring optimal resource utilization and service delivery.
- Provide ongoing support through comprehensive assessment, care planning, and evaluation of individual patient needs.
- Enhance quality management and promote continuity of care through effective case management, utilization review, and discharge planning.
**Requirements:**
- BLS certification through the American Heart Association (must be attached).
- Minimum of 2 years of recent RN Case Management experience.
- Experience in a LTAC or STACH setting, MCG, and discharge planning across acute care settings.
- Strong collaboration skills with an Interdisciplinary Team (IDT).
- Valid Texas RN or multi-state RN license (must be attached).
- Monday - Friday shift from 8:00 AM - 4:30 PM.
