**Exciting Opportunity for an RN Case Manager!**
**Job Description:**
- Join a dynamic healthcare team dedicated to optimizing patient care and resource utilization.
- Coordinate management of care to ensure compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation tailored to individual patient needs.
- Enhance quality of patient management and satisfaction through effective case management and discharge planning.
- Act as a patient advocate, addressing holistic needs and ensuring patient preferences are prioritized.
- Collaborate closely with interdisciplinary teams (IDT) in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
**Facility Requirements:**
- Must have BLS certification (American Heart Association) attached to submission packet.
- Minimum of 2 years recent RN Case Management experience.
- Experience in LTAC or STACH, along with discharge planning across acute care settings.
- Strong skills in MCG and IDT collaboration.
- Must hold a valid CA RN license (attached to resume).
- Shift: Monday - Friday, 8:00 AM - 4:30 PM.
