We are seeking an experienced Case Management Registered Nurse (RN) to join an acute care in Kealakekua, HI. In this role, you will coordinate comprehensive patient care plans, facilitate safe and timely discharges, and serve as a key liaison between patients, families, and the multidisciplinary care team. Key Responsibilities
• Assess patients' medical, psychosocial, and functional needs to develop individualized care coordination plans
• Collaborate with physicians, social workers, and interdisciplinary teams to facilitate appropriate level-of-care transitions
• Coordinate timely discharge planning to ensure safe transitions to post-acute or community-based settings
• Identify and address barriers to discharge, including social determinants of health and insurance authorization needs
• Conduct utilization review and monitor patient status to support appropriate admission criteria and length of stay
• Educate patients and families on discharge instructions, community resources, and follow-up care plans
• Document all case management activities accurately in the electronic health record in accordance with regulatory standards
✅ Requirements
• Active HI or Compact RN license
• ACLS, BLS/BCLS, PALS, NRP, Telecommunication, CPI, ENPC, RNFA, TNCC, AWHONN, NIHSS, STABLE, ASLS certifications
• 2+ years of Case Management experience
• Case Management certification (ACM or CCM) preferred
Pay
Pay range: $2,640 to $2,728 per week.
Travel Case Manager RN package: up to $2,728/week (40 hours) = $40/hour taxable + up to $1,128/week in travel reimbursements if eligible under IRS tax-home and expense rules.
Local package: $66/hour fully taxable ($2,640/week for 40 hours).
Overtime paid in accordance with applicable wage-and-hour laws and the assignment agreement. Actual compensation varies based on hours worked, assignment terms and travel reimbursement eligibility.
