Experience:
Minimum of 2 years acute care hospital Case Manager experience performing care coordination and discharge planning.
License:
CA state license required. Pending license accepted, but must be licensed upon start date.
Certifications:
Must have relevant certifications.
Must-Have:
Experience in acute care hospital case management is a must.
Must be able to perform care coordination and discharge planning independently.
Experience with assessing patients and knowledge of drug/alcohol abuse treatment options.
Ability to organize and participate in family meetings.
Experience with HMOs and arranging necessary services such as DME, SNF, and transport.
Description:
