Behavioral health experience does not apply.
Client experience is a must.
Travel experience required.
License:
Valid state nursing license is required. Pending licenses are accepted but must be licensed upon the start date.
Certifications:
No specific certifications required for this position.
Must-Have:
Experience with assessing patients, care coordination, discharge planning, and utilization review.
Ability to organize and participate in family meetings.
Experience with HMOs, IPAs, etc.
Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, and patients with mental health history or in crisis.
Competence in arranging durable medical equipment, skilled nursing facilities, and ambulance transport. Must be able to perform this task independently.
Description:
The position is for a Case Manager working in an acute care facility.
The shifts will include 4-5 weekend shifts in a 6-week period.
The case load may reach up to 25 patients.
The position requires participation in the development of treatment plans.
Knowledge regarding Centers for Medicare and Medicaid Services, Department of Health, and The Joint Commission/Core Measures/National Safety Goals will be beneficial.
The patient population includes neonates, adolescents, adults, and geriatrics.
Interdisciplinary support will include physical therapy and social services.
Travelers may be expected to float between nearby facilities.
Inflexibility beyond the contracted schedule may lead to termination of the contract.
All requests for time off must be communicated in advance.
Candidates are expected to commit to working during major holidays.
Two references covering one year are required during the compliance process.
